After 13 days in the hospital and feeling very fragile, I was nervous about coming home. The hospital said I would receive home health care after I arrived home. I was so thankful when each of your (Transitions At Home) folks arrived including Samantha, Amy, Briana and Dan. They were courteous, knowledgeable and so helpful to me! I made very good progress, and after 4 or 5 weeks I was able to be discharged. I still think of them as guardian angels. Please let them know how important their work is.

I am writing to let you know that your team of caregivers provided excellent care for my mom. I covered many overnights so have firsthand knowledge regarding the care mom received. I live in another town so having mom taken care of by such a great team was always so comforting.
Although mom has now passed, I can't say enough about the personal care and attention Kim Lopez and her team provided daily to mom. I know Kim went out of her way to safeguard mom. Kim contacted me frequently with updates or suggestions to keep her comfortable and safe as her health declined. I can't thank Kim enough for all the excellent care and supervision of staff she provided.
Other staff members like Rose Otis provided wonderful loving, one-on-one care. Everyone was competent, always upbeat and willing to do what they could to give mom the best possible quality of life. Kim is a remarkable person who was always on top of meds, staffing and anything mom would need.
I want to thank you for the great organization you have! I'll be forever grateful.
This home health agency is excellent. The nurses are all super helpful and always willing to answer questions anytime of the day. Dana in particular has been great, and has educated and guided us through the care that has been needed for our home.
