Our services help monitor and address medical needs and concerns following surgery, hospital or nursing home stays, inpatient rehab, or to aid in recovery from illness or injury. Please see our list of services or call us with your specific need or concerns and we will be happy to work on a personal care plan for you or your loved one.
How Home Health Care works:
Doctor’s orders are needed to start care. Once your doctor refers you for home health services, we schedule an in-home appointment to discuss your needs, ask questions about your health and design a care plan just for you.
Our staff will communicate with your doctor about your care and keep your doctor informed about your progress.
Remember that it’s important for our home health staff see you as often as the doctor ordered.
